Contracts

We respectfully request all employment agreements, appointment or offer letters, amendments, extensions, addenda, and personnel action forms setting the terms of employment for each individual currently employed and assigned in whole or in part to the men's basketball or football program in a health, performance, sports medicine, or sports science role, including any individual holding the title of Head Strength and Conditioning Coach, Director of Performance (or Sports Performance, Athletic Performance, or Human Performance), Head Athletic Trainer or Director of Sports Medicine, Physical Therapist, or Sports Scientist, or any variation of those titles. If complete agreements are not produced, we respectfully request, at minimum, each individual's base salary, all bonus and incentive compensation with the conditions triggering payment, and the contract term including start date, expiration date, and any renewal provisions. Personally identifying information such as home address, telephone number, date of birth, and banking or benefits information may be redacted.
